About Us:
We are a Minnesota assisted living home serving residents on waivered services. Our goal is to build strong relationships with referral partners and consistently fill available beds with residents who qualify under CADI, EW, or BI.

Role Summary:
This is not a hard sales job. This role is professional outreach, referral coordination, and relationship building with case managers, discharge planners, hospitals, shelters, and other relevant organizations. Your job is to help us build a steady pipeline of referral partners and schedule calls or tours when there is interest.
What You Will Do:
Outreach and partnership building
Research and identify Minnesota referral sources including case managers, discharge planners, hospitals, shelters, nursing homes, and other assisted living communities
Send professional outreach messages by email and LinkedIn using provided scripts
Follow up consistently and respectfully to build rapport over time
Keep notes on each contact’s referral process and preferences
Referral coordination and scheduling
Learn the basics of CADI, EW, and BI waiver placements and what we are looking for
Identify and prioritize partners most likely to refer waiver-eligible clients
Respond to inquiries promptly and route them to the owner when appropriate
Schedule calls, virtual tours, or in person tours on our calendar
Tracking and reporting
Track all outreach activity and responses in our lead tracker (Google Sheets or CRM)
Provide weekly updates on outreach volume, responses, and next steps
Maintain clean, organized records so nothing falls through
Requirements:
Strong written English and professional communication
Comfortable doing internet research and finding accurate contact details
Experience using Google Sheets and Google Docs
Organized follow up habits and attention to detail
Able to learn a process and improve it over time
Reliable internet and ability to communicate consistently
Nice to have:
Experience with healthcare, referrals, social services, or outreach roles
Experience with LinkedIn outreach and email follow up sequences
Important Boundaries
You will not handle protected health information or sensitive client documents
You will not make promises about admissions or placement outcomes
You will use our approved scripts and escalate anything complex to the owner
Hours and Pay:
Start part time: 10 to 15 hours per week
Pay: $6 to $10 USD per hour depending on experience
Opportunity to grow hours and rate based on results and reliability
Success Metrics (How We Measure Performance):
Number of new referral contacts added weekly with accurate information
Outreach messages sent and follow ups completed
Responses and conversations started with referral partners
Calls or tours scheduled
Quality of organization and tracking
